What do I need for an appointment?

For an appointment you will need a government-issued ID card (it can't be more than 3 years past the expiration date). You will need the completed document - no blank spaces. The party signing will need to be of sound mind; aware of what they're signing and willfully participating. Any required witnesses should be present with identification.If there are any special or extenuating circumstances, please include them in the appropriate box of the booking form when you book your appointment. If we are not able to fulfill the request, we will notify you immediately and a full refund will be provided.

What are the appointment charges?

Indiana does not allow charges of more than $10 per notarial act. 
A basic appointment includes:$45 Appointment Fee$35 Travel Fee$10 Notarization FeeIf you are having more than one document notarized, you may add additional notarizations when you book the appointment. You do not need to add anything if you only require a single notarization. It is included with the appointment.The appointment is scheduled for long enough to provide time to and from your location. The start time is the latest time your notary may be moving in your direction with the intention of concluding service by the end of the scheduled booking.

Should I pre-sign my document?

No. Do not sign your document in advance. Depending on the language contained in the notarial certificate at the end of your document, we may be able to notarize a signed document, but it would be better to wait until your appointment.

What are Special Circumstances?

The Special Circumstances box is there to allow you to add any relevant information you feel may be necessary to fulfill your appointment. If you have questions or general concerns, you may address them here. This will allow us the opportunity to address these concerns prior to the appointment, and also identify if there is a possibility that your appointment cannot be fulfilled. For instance, if you require a power of attorney for medical reasons, but the person granting POA is not conscious. Or, if there are other potential impediments that bring into question whether the individual signing is capable of understanding the document or willingly agreeing to sign it.This can also be for general instructions you feel are relevant, like where to enter a building or other details you feel are relevant to making your appointment run smoothly.
